Admissions Process

1. Schedule Tour 

Contact the School Office to schedule a Campus Tour and pick-up a Student Enrollment Application - (352)-336-5800

(2) Apply to SBLI

Submit the completed Enrollment Application click here and  follow all directions 

       

(3) Submit Documents

Submit a copy of all required documents to the School Office for review. You can email supporting documents to sbli.enrollment@gmail.com (Please make sure all documents are labeled with students name)

Required Documents for all Students

  1. Student’s Registration Contract Agreement (2020-2021)                         

  2. Birth Certificate                                                                                                   

  3. Social Security Card                                                                                            

  4. Shot Record (Form: 680)                                                                                          

  5. Physical Exam (Form: DH 3040) (Completed within the past 12-months)       

  6. Primary Parent’s Driver License
